Whether you’ve been referred by your healthcare provider or are interested in learning more, we’re here to help you take the next step toward preventing Type 2 diabetes.

The National Diabetes Prevention Program (NDPP) is a yearlong, evidence-based lifestyle change program designed to help adults at increased risk of developing Type 2 diabetes reduce their risk through sustainable changes to nutrition, physical activity, and other healthy habits.

Participants learn practical strategies for making lasting lifestyle changes, including building healthy eating habits, increasing physical activity, setting achievable goals, managing challenges, and staying motivated.

The program lasts one year and includes a series of sessions that build on one another to support gradual, sustainable behavior change.

The primary goal is to reduce the risk of developing Type 2 diabetes by supporting sustainable lifestyle changes. Weight management may be one component, but the program also focuses on nutrition, movement, goal setting, and long-term health.

Participants generally must be at least 18 years old, meet the program's BMI requirement (25 or higher, or 23 or higher for Asian American individuals), have no previous diagnosis of Type 1 or Type 2 diabetes, and not be pregnant at enrollment. Additional criteria must be met to establish increased risk of Type 2 diabetes.

Eligibility may be established through a recent blood test in the prediabetes range, such as a history of gestational diabetes, or a qualifying score on the CDC Prediabetes Risk Test, among other accepted criteria. Common qualifying results include an A1C of 5.7%–6.4%, fasting blood glucose of 100–125 mg/dL, or a two-hour glucose result of 140–199 mg/dL.
