To order a Utah birth or death certificate (or marriage or divorce record) online, in person, or through the mail, the following items are required:

Ensure you have current, acceptable identification.
Ensure you have documents proving your legal relationship to the person listed on the record.
Proof of relationship may include a marriage license or chain of birth certificates showing your familial connection to the person whose certificate you are ordering.
For example, if you are ordering a record for "Person A" and:
