Order a Vital Record Online

Using the State of Utah's online portal, SILVER. If you live in Utah, your vital record order will be routed through your local health department.

Ensure you have current, acceptable identification to prove your identity or your relation to the subject of the record.

Acceptable Identification List

Bring your completed order form, identification, and payment to one of our office locations. Vital records offices accept cash, check, money order, Visa, and Mastercard.

We will process your order while you wait.

Copy both sides of your current, acceptable identification to prove your identity or your relation to the subject of the record (enlarge for ease of reading, if necessary).

Acceptable Identification List

Write a check or money order for the appropriate amount to:

We cannot accept credit cards for orders through the mail.

Mail your completed order form, copy of your identification, and payment to the location listed below.

Be sure to include ATTN: VITAL RECORDS on the envelope.

We promptly process and return orders through the mail.

We cannot process orders without:

  • the correct payment
  • a fully completed order form
  • a legible copy of the requester's ID
