How To Apply for a Passport

To apply for a passport, you must schedule an appointment and bring all required documents to your appointment, including two separate payments (one to the U.S. Department of State and one to the Clerk's Office).

You should apply in person if:

  • You have never been issued a U.S. passport
  • Your previous passport was issued when you were under age 16
  • Your most recent passport was issued more than 15 years ago
  • Your passport was lost or stolen

Make sure to review the required documents and fees before your appointment to avoid delays in processing.

Passport applications are accepted by appointment only.

Step 2: Gather the Required Documents

    • Payable to Salt Lake County Clerk

For 1-2 day delivery after the passport is processed at the U.S. Department of State, pay $21.36. Include this fee with your check or money order payable to the U.S. Department of State. Only available for passport books mailed to addresses within the United States.

Step 3: Bring Your Documents to Your Appointment

Bring the following documents to your appointment:

You must provide a valid photo identification document (ID) when applying in person if you are:

  • Over age 18
  • A parent or guardian applying with your child

  • Photos must meet all State Department requirements.
  • Passport photo service is available in our office for applicants aged 5 years and older for a fee of $10.00

  • Documents not acceptable include:
    • hospital certificate
    • baptism certificate
    • notification-of-birth
    • California abstracts of birth.
  • If born outside the U.S. you must provide a Certificate of Citizenship/Naturalization or Report of Birth Abroad.

Must be provided for all passport applicants.

Step 4: Bring Your Documents to Your In-Person Appointment

  • Please bring all required documents to your scheduled appointment.
  • ALL APPLICANTS must personally appear when applying for a passport.
    • Children under 16 years of age must have both parents present.
    • Children 16 or 17 years of age must have one parent present.

Step 5: What to Expect After Your Appointment

Application processing times may vary, so please refer to the U.S. Department of State website for the most up-to-date information.

Passport Processing Times:

  • Routine: 4-6 weeks*
  • Expedited: 2-3 weeks*

*Processing times only reflect the time your application is with a passport agency or center. It may take 1-2 weeks for your application to arrive before processing begins.
