Final Local Entity Plat (FLEP)

During the 2025 first special session, the Utah State Legislature passed SB1006. This modifies and clarifies the process of certifying the local government changes that affect or create local government boundaries.

FLEP Review and Recording Process

  1. The entity or surveyor of record will email a digital copy of the proposed Final Local Entity Plat (FLEP) to the County Surveyor at surveyor@saltlakecounty.gov subject line "FLEP Review" and pay a $200.00 plat review fee
  2. The County Surveyor's office will review the submitted FLEP and respond via email to the entity and/or surveyor of record with feedback on any needed corrections or approval
  3. The entity and/or surveyor of record will correct and resubmit a digital copy to the County Surveyor staff if changes were requested
  4. After final approval of the FLEP, recommend the entity or surveyor of record plot the proposed FLEP on mylar
  5. The surveyor of record will sign, date, and seal the FLEP
  6. The entity (or entities, if applicable) will approve and sign the FLEP
  7. An appointment is required for County Surveyor's signature of the FLEP. Call or email Melody Baugh @ 385-468-8240 mlbaugh@saltlakecounty.gov to schedule.
  8. The Salt Lake County Surveyor will:
    • Approve and sign the mylar
    • Scan the plat
    • Email a digital copy of the plat to the entity or the surveyor of record who prepared the plat
  9. The County Surveyor will return the mylar plat to the entity, or surveyor of record at the time of signature
  10. The entity will email the scanned copy of the mylar plat, along with digital copies of the notice of impending boundary action and resolution(s) to the Lieutenant Governor via their webpage
  11. The Lieutenant Governor will issue a certificate of annexation or a certificate of creation
    • The entity will advise the Lieutenant Governor where to mail the certificate of annexation/creation; or
    • The entity will arrange to collect the certificate of annexation/creation at the office of the Lieutenant Governor.
  12. The entity will record the original FLEP documents in the Office of the Salt Lake County Recorder
    • FLEP documents include the certificate of annexation/creation, the notice of impending boundary action, the resolution(s), and the mylar plat.
  13. The County Recorder will record the documents.
  14. The Final Local Entity Plat process is completed.

Utah Code 17-61 outlines the duties of the Lieutenant Governor, County Surveyors, and County Recorders in the process of certifying local government boundary and name changes.

Utah Code 17-73-507 outlines duties of the county surveyor to review and approve any boundary action that establishes, modifies, or eliminates the boundary of a local entity, including the incorporation, annexation, withdrawal consolidation, division, boundary adjustment or dissolution of the local entity.
