Apply for a Regulated Business Permit

Your health permit is separate from your:

  • business license from your city
  • Utah Division of Occupational and Professional Licensing (DOPL) license (if applicable to your industry)

Permits are not transferable; when a change of ownership occurs at a permitted facility, the new owner must apply for a new permit and pay all applicable fees.

You must pay the application fee before we will schedule your preopening inspection or issue your permit.

How to get a regulated business permit:

Fully complete and submit a permit application (available in "Resources" above). Be sure to sign the application.

Use only Adobe Acrobat Reader to complete and submit your application. Web browser PDF viewers will NOT properly submit your application.

You may also print the application, complete it by hand, then scan or photograph the completed application and email it to the address at the top of page 2 of the application.

After you submit your completed application, wait until the next business day and call 385-468-3835 to pay the permit fee.

We accept Visa or Mastercard over the phone.

You may also mail a check or money order, or pay in person with cash, check, money order, Visa, or Mastercard.

The area inspector will contact you within three business days of your payment to schedule your pre-opening inspection.

Prepare for your pre-opening inspection by reviewing the requirements in the Salt Lake County Health Regulations applicable to your industry.
