The Salt Lake County Health Department offers required and recommended immunizations for children, adolescents, and adults.
2025-26 COVID Vaccine Available October 1!

We expect to have COVID vaccine available on October 1, 2025. You may call now to schedule a COVID vaccine appointment for October 1 or later.
We will have COVID vaccine for people 6 months and older. A prescription will NOT be necessary.
- People 65 and older may receive the vaccine without attesting to any health conditions.
- People 6 months to 64 years old must sign an attestation that they have an underlying health condition, such as those listed here. You will not need to specify which condition(s) you have.
- Asthma
- Cancer
- Cerebrovascular disease
- Chronic kidney disease
- Chronic lung disease
- Chronic liver disease
- Cystic fibrosis
- Diabetes (type 1, type 2, gestational)
- Disabilities, including Down syndrome and cerebral palsy
- Heart conditions
- HIV
- Immunocompromised status
- Mental health conditions, including depression
- Neurological conditions, such as dementia or Parkinson's disease
- Obesity
- Physical inactivity
- Pregnancy or recent pregnancy
- Sickle cell disease
- Smoker, current or former
- Solid organ or blood stem cell transplant recipients
- Tuberculosis
The self-pay price for COVID vaccine ranges from $105 to $158, depending on vaccine brand.
We will bill our accepted health insurance plans, but please contact your specific plan to confirm they will cover the cost of the vaccine. You will be responsible for paying claims denied by your insurance plan.
Through the Vaccines for Children (VFC) program, we will have low-cost COVID vaccine for uninsured children 6 months to 18 years old pending the CDC's adoption of ACIP's recommendation made September 19. The cost is $10 if paid at the time of service or $14.50 if billed.
We do not currently have low-cost COVID vaccine available for uninsured adults 19 and older.
