Diabetes is on the rise. Fortunately, the Healthy Living Team can help you learn more about the programs and resources available to prevent, delay, or manage diabetes. We help individuals access education and support in a variety of ways, including:

We also work with healthcare clinics, employers, and community-based organizations. Some of the services we provide include:

  • Assist healthcare clinics to increase screening for prediabetes, diabetes, and chronic kidney disease.
  • Help employers and community-based organizations get set up to offer the National Diabetes Prevention Program, through technical support and funding (when available).
  • Support existing National Diabetes Prevention Program providers.

National Diabetes Prevention Program (National DPP)

NATIONAL DIABETES PREVENTION PROGRAM

The National Diabetes Prevention Program is a nationwide initiative aimed at reducing the risk of type 2 diabetes through its evidence-based lifestyle change program, Prevent T2.

Prediabetes Risk Test

Find out if you are at risk of developing type 2 diabetes by taking the short prediabetes risk test. A person with a score of 5 or higher is at risk of having prediabetes. However, only a blood test can determine an official diagnosis.

Diabetes Self-Management Education and Support (DSMES)

Diabetes self-management education and support provides personalized recommendations to help you manage diabetes. You will work with a diabetes care and education specialist who will teach you practical skills to fit diabetes care into your life.
